As part of the application process you’ll be asked for the following. Please to be sure to have them available before beginning the application process.

• Mini-Grant Fund Request Please fill this out and save it before starting the application process. You’ll be asked to upload this file at the end.
• You will be asked to upload a sample flyer for your event
• You will be asked to upload an agenda for your event
• A link to your Organization 990 Form
• A copy of the the Organization 2016 Operating Budget
• Maryland SDAT Department ID (Must be searchable at http://dat.maryland.gov/)
